Default Stuck in reverse / stuck in first - at the same time

In all the years I've been messin' with cars this never happened, but surprising to me, I'm told quite common. My 4 speed tranny was in R and 1st at the same time. I had to remove the shifter linkage, and reverse column lock, and move the R tranny lever by hand. Got it out of R, back in N, reattached the linkage, and now it's back to N- N for Normal.

What type of shifter do you have? some Hurst and other brands of shifters need adjusting, it's easy to do, just takes a small drill bit inserted into the adjusting hole to align it up.
    Don't touch the trim!

Thanks, GG. It's an OEM shifter, and it's in adjustment. I've adjusted it, or shifters in general, hundreds of times. That's what makes this so weird.
